Special Cremation Forms
The cremation forms and documents on this page will assist you in completing the documents necessary to arrange a direct cremation. Please read the instructions carefully before proceeding. If you have any questions about a form or how to complete a form, call your Affinity Cremation representative before proceeding at (813) 684-7500.
Each form is provided in PDF format which requires Adobe® Reader® to open and print. We recommend using Adobe® Reader® version 7.0 or greater for these forms. If you do not already this free software, it is available by clicking on the logo below.
For your convenience these documents are designed to allow you to fill in any required information before printing. Simply click your mouse in the form area where you wish to insert information, type the information and then print the document. You may also simply print the blank form to be filled in by hand after printing.

Special Cremation Forms
The following are special forms that are sometimes needed to arrange for our services. Since they are not always required, we have placed them on a separate forms page to lessen confusion. You should only use these forms if you have been advised to do so by an Affinity representative.

Hillsborough Medical Examiner Body Release Form (PDF - 28K)
This form is required by the Hillsborough Medical Examiner Department in order to release the body to us. It must be signed by the next of kin and witnessed. The form should then be faxed back to us for completion before we forward it to the Medical Examiner Department. The PDF form may be completed before or after printing from your computer and faxed to us as soon as you are able.

Assignment of Authority Form (PDF - 223K)
This form may be used to assign the authority to arrange the cremation to someone other than the legal next of kin. This form would need to be signed by the legally authorized person(s). We will need to obtain the signature(s) on this form before proceeding. This form must be witnessed by a Notary Public (or equivalent).
